Tennessee Woman Survives Two Lethal Injection Doses: How Does It Work, and How Often Do Executions Go Wrong?
October 5, 2026
05:25
Christa Pike survived an attempted execution in Tennessee after officials administered two doses of pentobarbital but failed to end her life, an extraordinary breakdown that has prompted the state to suspend its remaining executions for 2026.
Pike, 50, was scheduled to become the first woman executed by Tennessee in more than two centuries. The execution was carried out at Riverbend Maximum Security Institution in Nashville on September 30, but the procedure was ultimately halted and Pike was transported to an off-site medical facility.
Her lawyers later said she remained alive after the two injections. Media witnesses also reported hearing her breathing and snoring during the prolonged procedure.

Tennessee Gov. Bill Lee subsequently ordered a comprehensive third-party review of the state’s execution process and said the remaining scheduled execution this year would not go ahead.
The incident has raised a basic but difficult question: how is lethal injection supposed to work, and how often does the process fail?
Pike was convicted of the 1995 murder of 19-year-old Colleen Slemmer, a fellow Job Corps student in Knoxville. She was 18 when the crime occurred.

After years of legal proceedings, Tennessee scheduled her execution for September 30, 2026. The U.S. Supreme Court allowed the execution to proceed after a lower federal court had temporarily blocked it.
Officials began the lethal-injection process at the prison, but the procedure did not produce the intended result.
According to court filings and witness accounts, Pike remained alive after two doses of pentobarbital had been administered. She was eventually removed from the prison and taken to a medical facility.

Her condition was not immediately disclosed publicly.
The Tennessee Department of Correction said the execution team followed the state’s established protocol. The department has not publicly identified the precise reason the drugs failed to produce the intended outcome.
Tennessee currently uses a single-drug lethal-injection protocol based on pentobarbital.

Pentobarbital is a barbiturate that depresses activity in the central nervous system. In medical settings, drugs in this class have been used for purposes including sedation and seizure control.
In an execution setting, a very large amount is intended to produce profound unconsciousness followed by respiratory and cardiovascular failure.
Tennessee moved to a single-drug pentobarbital protocol after a broader review of its execution procedures. The state’s Department of Correction completed a revised protocol in December 2024.

The change reflected a wider problem in the United States: obtaining drugs for executions has become increasingly difficult as pharmaceutical manufacturers have restricted their distribution for capital punishment.
There is no single lethal-injection formula used throughout the United States.
Some jurisdictions have used three-drug protocols involving a sedative or anesthetic, a paralytic drug and a drug intended to stop the heart. Other jurisdictions use a single drug, usually an anesthetic or sedative at an extremely high level.

Tennessee’s current protocol is based on the single-drug approach.
The intended sequence is relatively simple in principle. The drug is administered intravenously, produces unconsciousness and then causes the body’s vital functions to fail.
The crucial point is that an execution depends on more than the drug itself. The state must also establish reliable intravenous access and administer the correct substance under its prescribed procedure.

When something goes wrong at any stage, the process can be delayed, interrupted or produce an unintended medical outcome.
There is no single explanation for every failed execution.
One potential problem is difficulty establishing intravenous access, particularly in prisoners whose veins are difficult to access. This can result in prolonged attempts to place an IV line or the need to find another access point.

Drug-related issues can also matter. States have faced shortages, changes in suppliers and questions surrounding the quality or availability of execution drugs.
There can also be procedural complications. An execution protocol may specify what happens when the first attempt does not produce death, but there may be less clarity about what should happen after repeated failure.
Pike’s case has brought that issue into sharp focus. Her attorneys have alleged that Tennessee’s protocol did not provide a meaningful medical-rescue mechanism after the execution failed.

Death-penalty expert Robin Maher of the Death Penalty Information Center described the incident as unprecedented in modern U.S. execution history because Pike remained alive after two rounds of lethal-injection drugs.
The case was also notable because it followed another failed execution attempt in Tennessee only months earlier.
In May 2026, officials halted the execution of Tony Carruthers after they were unable to establish the necessary IV access.

That means Tennessee encountered two separate execution failures involving lethal injection in the same year.
The incidents have prompted renewed attention to the reliability of the state’s execution procedures.
There is no single universally accepted modern statistic for the frequency of botched executions, partly because researchers use different definitions of what counts as a failed or botched execution.

One of the most frequently cited studies comes from legal scholar Austin Sarat, whose 2014 book “Gruesome Spectacles: Botched Executions and America’s Death Penalty” examined executions in the United States between 1890 and 2010.
Sarat found that 276 of 8,776 executions during that period went wrong in some way. That works out to about 3.15 percent overall.
Lethal injection had the highest botched-execution rate among the methods examined in that historical dataset, at about 7.2 percent.

That div should not be presented as the current failure rate for U.S. lethal injections. It covers executions over a 120-year period and includes historical procedures that differed substantially from modern protocols.
Recent failures, however, demonstrate that execution problems have not disappeared.
The term does not have one universally accepted legal definition.

Sarat’s research generally treats an execution as botched when there is a breakdown or departure from the intended execution procedure, including unexpected complications or delays that can cause additional suffering or reflect serious procedural failures.
Examples throughout U.S. history have included problems with hanging, electrocution, lethal gas and lethal injection.
In lethal-injection cases, problems can include difficulty establishing IV access, unexpected reactions, equipment failures, delays or incorrect administration.

The United States has used several methods of execution, including hanging, electrocution, lethal gas and firing squads.
Lethal injection became the dominant method during the modern era. Since 1976, it has been used for the vast majority of U.S. executions.
The Death Penalty Information Center currently lists lethal injection as authorized in 28 states, as well as by the federal government and U.S. military. Several jurisdictions also authorize alternative methods.

That has forced correctional authorities to seek alternative suppliers, different drugs or new drug combinations.
In some cases, states have turned to compounding pharmacies, which can operate differently from major pharmaceutical manufacturers.
These developments have made execution protocols more complicated and, in some cases, more difficult to verify independently.

The legal consequences can be complicated.
An execution is carried out under a specific death warrant, court orders and state procedures. When an execution attempt fails, authorities must determine what can legally happen next.
Pike’s situation is particularly unusual because she survived the administration of the execution drugs and was taken for medical care.

The governor said carrying out a lawfully imposed death sentence is one of the state’s most serious responsibilities and said the process must be legal, constitutional and effective.

Tennessee’s move follows an earlier review of its execution process. In 2022, Lee paused executions after the state identified problems with its procedures. A subsequent review eventually resulted in a revised protocol.
Pike’s case has now triggered another examination.
Several critical questions remain unanswered.

Investigators must determine why the two doses of pentobarbital did not produce the intended result, whether there was a problem with intravenous access or drug administration and whether the state’s protocol was followed exactly.
They must also determine what happened medically after the drugs were administered and whether anything in the execution procedure contributed to Pike’s continued survival.
The condition of Pike after she was taken to the medical facility was not immediately made public.

Until the state review and any related legal proceedings are completed, claims about the precise cause of the failure should be treated as allegations or possibilities rather than established facts.
Pike’s case has renewed scrutiny of lethal injection because it combines several problems that have appeared separately in past executions: the challenge of administering drugs reliably, the secrecy surrounding some execution protocols and uncertainty over what should happen when the intended procedure fails.
The historical data also show why the word “botched” is not limited to rare sensational cases. Researchers have documented execution failures across different methods and periods in U.S. history.
What makes Pike’s case particularly striking is that the state administered two doses of its designated execution drug and still did not complete the execution.
The independent review now ordered by Tennessee will be important in determining whether the failure resulted from an isolated medical or technical problem, a flaw in the state’s protocol or a combination of factors.
Until those findings are released, the most accurate conclusion is also the simplest one: Tennessee attempted to carry out an execution, the procedure failed, and authorities are now investigating why.
